Lady of the Year Application

	LADY OF THE YEAR 2011-2012. SUBMIT BY MARCH 31, 2012 Now is the time to give credit to that special Lady who is the Right Arm of your Auxiliary. The Missouri State Council wishes to HONOR an outstanding Auxiliary member at the State Convention. However, it will be up to each Auxiliary to select their Honoree and to fill out her qualifications on the form below and to submit a short summary of her accomplishments for the last two (2) years, from March 1, 2010 thru March 1, 2012. We understand that this will be a difficult decision as there are so many ladies that give 100% to everything that they do, but we are looking for that special LADY. Please submit this form to the Ladies Auxiliary Chair-couple no later than March 31, 2012. This gives us time to judge the entries, notify and make arrangements for the LADY OF THE YEAR to attend the Convention and to have her plaque made. We appreciate your help and consideration in this matter. Please do your nominee the courtesy of taking a few moments to thoroughly evaluate her achievements for the past two (2) years before submitting her for this award.
